The Foundation requires a brief Mid-year Update and a Year-end Evaluation. Please take a few minutes to reflect on your work and to update us on your progress. Please share with us both your successes and your challenges.

MID YEAR UPDATE DUE FEBRUARY 1 We request that you send us a brief update on the progress of your current project by February 1. This is an absolute necessity of you are applying for continued funding during the following grant cycle. Please by sure to address the following:
- Progress in meeting the matching funding requirement - Whether the grant funds are being spent as indicated in your original grant - Challenges you have encountered that have forced you to make changes in your original project - Highlights of interest regarding the organization or the project currently funded by The Chapman Foundation

YEAR END EVALUATION DUE JUNE 30 Please submit a brief evaluation signed by the CEO of your organization. We require that this be submitted prior to being funded for a future grant.
Be sure to include contact information for the person preparing the evaluation, as well as detailed answers to the following:
1. Was the grant used as originally agreed in the grant agreement? If not, please explain? Specifically how were the grant funds used?
Please outline the specific outcomes of your project and how you measured them:
2. Number of students served 3. Area of residence and demographics of students served 4. How are students selected for your program? 5. How did your program make a difference for these students? 6. How did it improve the student's likelihood of attending and succeeding in college? Please provide us with as much data as you have available about college attendance and success for those who you serve.
